Sullivan County Health Department has received 100 doses of Moderna Covid-19 Vaccine for which we will be hosting a clinic.

We will be targeting the following groups: PHASE 1A-PATIENT-Facing Health Care Workers and Long-Term Care Facility Residents and Staff, Phase 1B-Tier 1: First Responders, Emergency Services, and Public Health Infrastructure and -Phase1B-Tier 2: High-Risk Individuals

The clinic will take place on Friday February 5th, 2021 at North Central Rural Electric Co-op in Milan, MO. Those being vaccinated will need to FOLLOW COVID-19 VACCINE SIGNS INTO their Shop Building-9:00 a.m.-11:00 a.m.

Members of these target groups must Pre-Register for the clinic by calling one of our registration phone lines below. Registration WILL BEGIN ON WEDNESDAY-February 3, 2021 FROM 10:00 a.m.-11:30 a.m.:

Those Registered will need to Print and Complete the vaccine consent form YOU MAY GO TO SULLIVAN COUNTY HEALTH DEPARTMENT’S WEBSITE  schdmilanmo.com; CLICK ON “COVID-19 OUTBREAK” LOOK UNDER RESOURCES, AND CLICK ON COVID-19 VACCINE CONSENT FORM . PLEASE PRINT AND COMPLETE FORM PRIOR TO CLINIC AND BRING IT WITH YOU AND UNDER EVENTS IS THE DETAILS OF THE PRE-REGISTERED CLINIC ONLY. Those registered will also be assigned an ID# to write on your consent formNo one will receive a shot without their correct ID# and completed consent form. Phone Lines will be busy so please exercise patience.  These three phone lines are to be used ONLY by those in the previously described target groups, not the General Public.  Please note that people who are receiving antibody therapy must wait 90 days before receiving a COVID-19 vaccine AND PLEASE DO NOT REGISTER IF YOU ARE SICK.  ADDITIONAL COVID-19 RESOURCES ARE AVAILABLE ON OUR WEBSITE.PLEASE BE PATIENT AND SAFE!  COVID-19 PRECAUTIONARY MEASURES WILL BE IN PLACE DURING CLINIC AND WE WILL REQUIRE MASKS UPON ENTRANCE, IF YOU DO NOT HAVE ONE, ONE WILL BE PROVIDED FOR YOU!!  THANK YOU
