What is a community health assessment?

“A community health assessment identifies the strengths and resources available in the community to meet the needs of children, youth, and families. The assessment focuses on the capabilities of the community, including its citizens, agencies, and organizations. It provides a framework for developing and identifying services and solutions and building communities that support and nurture children and families.”

You are invited to participate in the Sullivan County Health Department’s Community Health Assessment Survey. If you live in Sullivan County, we would appreciate your input. The survey takes 10 minutes or less to complete, and your responses will help the Health Department better understand the needs, concerns, and priorities within out community.

Your feedback plays a vital role in shaping the future of local health services and programs in our community. By completing this survey, you are helping us identify key health concerns, improve our decision-making and program-planning activities, encourage community involvement, and strengthen our ability to secure additional funding and resources for Sullivan County. The information gathered will also support our efforts toward public health accreditation and help us continue working to improve the health and well being of everyone in our community.
