Permit Applications

Permit applications are available from the state and assistance can be found at our office during normal business hours, or if you are making an application for an Onsite Sewage Disposal System or repair of an existing system you will need to submit the following items to the Department of Health and Sr. Services in Jefferson City, Missouri.

• A copy of the Soil Report from a Registered Soil Evaluator
• A site diagram of the proposed system including setback distances, elevations, and slope
• Permit fee
