Vital Records

Sullivan County Health Department provides vital records for only the state of Missouri. If you would like to obtain a copy of a birth or death certification, please complete an application and submit it in person at the health department. And remember to bring the required fee! The entire in-person process is very brief and will likely not take any longer than ten minutes. Please make sure to bring an ID when you come to our office.

If you cannot come to our office in person, you also have the option to submit a notarized application via mail.

• Birth Certificates are $15.00 each
• Death Certificates are $14.00 for the first copy and $11.00 for each additional copy
